The Title Tag and the amount and quality of incoming Links to your site.

One is on-page optimization = Title Tag

and one is an off-page form of search engine optimization, Incoming Links.

The on-page optimization you have total control over. The Title Tag for instance. The Title Tag should contain the keyword or keyword phrase that that particular page is being optimized for.

Always enclose the title in an <H1>TITLE</H1> and use the main keywords in the first paragraph and liberally throughout the page content.

Keyword density for example, some believe that if you over use your keywords Google will hit you with an over optimization penalty. Others claim that no such penalty exists.

Theories being what they are (Unproven educated guesses) are not something to base your optimization practices on. I recommend seeing what the top ten sites are doing, analyze their keyword density and base your keyword density optimization on the results of that research.

Off-page optimization is harder to control.

It is an accepted fact that the anchor text that's the blue underlined part of an incoming link is very important. It tells the search engine what your page is most likely about.

This gives your page a "Reputation." This is why CNN ranks #1 for the keyword "News" and the word news is not in it's title tag and is only on the page maybe once or twice.

This tells us that what the anchor text of an incoming link says about a site is viewed to be very important by Google. Most other sites link to CNN by putting the word news in the anchor text of the link pointing to the CNN site.
